The nearest hospital and urgent care, with driving time
The police non-emergency line
Where the water shut-off and gas shut-off are, and how to reach them
Your phone number, or a local contact who can be there

Getting it right

Put it on the fridge door or inside a kitchen cabinet door, where anyone would look.
Walk to the shut-offs yourself and describe the route the way a stranger would need it.
Check the hospital hours. An urgent care that closes at 8 PM is not the answer at midnight.
Many cities and insurers expect emergency information to be posted. Check your local rules.
